What to Provide for a Case Review

To make sure we can provide an appropriate referral and evaluation, we request the following for review:

  • A clinical summary or clinical notes.
  • Relevant/most recent imaging studies such as CT scans, MRI scans, x-rays, and angiograms (Actual films/scans must be available at the appointment since accurate clinical assessment cannot be based on the dictated, paper report.)
  • Insurance and demographics information
