To provide the highest quality of medical care and expertise, Upstate doctors each offer unique clinical specialties. To make an appointment please refer to the Physicians/Staff pages to find the physician that best meets your needs.

Please note that due to the current SARS-Cov-2 (COVID 19) disease, we have implemented protocols for in-person visits, including wearing of masks while in the office.  Our mission is to keep you and our staff safe and curb disease spread in our community.  We offer and suggest tele-medicine appointments for established and new patients.

To request a referral, consultation, or second opinion, you may directly contact the physician that practices the specific subspecialty needed. If you are unsure as to which physician you should contact, please call our main department number and our staff will assist you.

What to Provide for a Case Review

To make sure we can provide an appropriate referral and evaluation, we request the following for review:

  • A clinical summary or clinical notes.
  • Relevant/most recent imaging studies such as CT scans, MRI scans, x-rays, and angiograms (Actual films/scans must be available at the appointment since accurate clinical assessment cannot be based on the dictated, paper report.)
  • Insurance and demographics information
